Permission to Take Medication at School 

Get your child’s doctor to complete this form so he or she can take medications.  

Your child may need to take medication at school, so you'll want to have clear instructions from your child's doctor to share with the school nurse. 

This form can help you provide the information needed to arrange for your child to take medications at school. It is possible that your child’s school may require you to use its version.
